Access to practice information
All patients are entitled to have access to their medical records with a few exceptions. Copies of medical records may be made available although there will be a charge. Please ask at reception for further information.
It is important that we retain your trust as a patient and maintaining patient confidentiality is part of this. For this reason only you as the patient will be given information on test results or appointments made. If you telephone for results you may be asked for proof of identity by confirming personal information. If a relative or employer contacts us for information of an appointment we will not be able to give that information to them. To maintain your confidentiality identifiable patient information will only be shared within the practice team and to clinicians to whom you may be referred. Disclosure of identifiable information to any other outside agencies will only be done after receiving written permission from the patient.
Comments and Complaints
We treat as a complaint any expression of dissatisfaction with our service which calls for a response. We listen to your complaints, treat them seriously, and learn from them so that we can continuously improve our service. If you would like any further information on how to make a formal complaint or contact numbers for the Patient Advisory Liaison Service, please ask a receptionist for a copy of our complaints procedure.
Rights and Responsibilities
We provide emergency medical care so if you need to see a Doctor urgently you will be seen. We also endeavour to provide access to a Doctor or Practice Nurse on the same day through our Triage system. We are a teaching practice and you may be asked if your consultation can be videoed or if a student can observe. You may choose not to participate in the training and you consultation and care will not be affected. We ask that you be as prompt as you can for an appointment, if you are more than 10 minutes late you will need to re-book. We will treat you as an individual and respect your privacy and dignity. In return we ask you to be polite to all our staff. We will if necessary remove you from our practice list if you behave unreasonably or are abusive to our staff on the premises.
